1). The client expressly agrees that Maxwell Healthcare Ltd may charge a cancellation fee of 70% of the total cost of the Influenza Vaccination if:

a) A confirmed booking is cancelled at any time within two weeks prior to the appointment date of vaccination, or
b) The patient does not attend the vaccination appointment at the pre-booked time.

2). A minimum fee of $280 plus GST / visit / per session / per site is applicable only when less than 10 vaccinations are given.

3). Maxwell Healthcare Ltd invoice is payable within 7 working days.
